Upon submission of a case, you are acknowledging that you have read and accepted Atelie Digital Dental Solutions’ warranty, re-make policy, and payment terms.

 

WARRANTY

 

Atelie Digital Dental Solutions warrants that all custom dental prosthesis and products are made according to the clinicians’ specification and fit the cast provided by your impression or scan (physical or digital). If the prosthesis or product fits the model but not the mouth, there is a discrepancy in the impression or scans provided and the lab will not be responsible. Subject to the return of the restoration or product that is placed and then fails, Atelie Digital Dental Solutions will warranty the restoration as follows:

  • Zirconia crown – 5 years
  • Emax crown and veneers –2 years
  • Denture–1 year
  • Hard nightguard–6 months
  • PMMA–no warranty due to the temporary nature of the product
  • Hybrid zirconia-–2 years ***
  • Hybrid acrylic—2 years ***
  • Titanium bar–2 years ***

***Warranty only valid if following Atelie Digital Dental Solution’s Hybrid Denture Protocol

Any implant assisted or supported prosthesis is not warrantied due to implant failure nor do we warranty implant parts or components. Please refer to manufacturer’s warranty for the corresponding implant system used.

Implant components such as transmucosal abutments (I.e. Multi-unit, low profile, etc.), titanium cylinders, ti-base, and prosthetic screws are not warranted by Atelie. These items are subject to the warranty of the manufacturer.

 

 

REMAKE POLICY

 

Fixed and removable remakes will be at no cost to the clinician if the re-make is due to error on behalf of Atelie Digital Dental Solutions. In this case, all of the original case materials, including models and impressions along with the dental restoration and a written reason, must be received by Atelie within 30 days of the invoice date to waive the fee

Warranty and re-make policies will not apply under the following conditions:

  • If the original appliance is not returned at the time of remake request.
  • If a remake or credit is requested greater than 30 days from invoice date.
  • If a remake or credit is requested after the lab deemed the provided case materials incomplete and/or unsatisfactory and doctor elects to proceed with the completion of the case without making any adjustment, refuses a try-in, or does not supply requested materials.
  • If a remake is requested because the doctor or patient requests a tooth shade different from the original request.
  • If a remake or credit is requested for a finished product due to poor fit, after try-in was approved or if complete dentures were fabricated without a try-in
  • If a remake is requested due to treatment plan or material change from original request.
  • If a remake or credit is requested because the appliance fits the model but does not fit in the mouth.
  • If repair is needed resulting from accident, neglect, abuse, failure of supportive tooth structure or tissue structures, non-compliance with occlusal guard recommendations
  • If improper oral hygiene by the patient causes restoration failure
  • If prosthesis is partially fabricated or completely fabricated by another lab other than Atelie Digital Dental Solutions
  • For hybrids, if the lab’s screw-retained hybrid denture protocol was not followed, warranty and re-make policy is voided
  • If the restoration was not seated by the same doctor who sent the case for fabrication.
  • If we inquired about the margin, impression or scan, but were instructed by the doctor or doctor’s staff to proceed with what we have.
  • If photos are requested and declined.
  • If bridges where the pontic site(s) had not been extracted or allowed sufficient healing time prior to the impression being taken.
  • If changes are made after Exocad designs were approved and production of case has begun.
  • If a case cancellation occurs during the fabrication process, Atelie will stop production and only charge the costs incurred up to that point. Refund will be made in the form of lab credit.
 
 
 

PAYMENT TERMS

 

Client will receive an invoice via Seazona for every case delivered detailing the products and associated fees incurred. Your obligation for payment will be due within 15 business days of receiving a monthly statement. A finance charge will be assessed to any past due account of 10% per month. Any account not paid within 15 days of the statement date may be put on hold and may be sent to collection. Client shall pay all costs of collection, including without limitation, collection agency and attorney’s fees, incurred by Atelie Digital Dental Solutions. Any payments made by check will incur a $50.00 fee for all returned checks. Atelie’s re-make policy will not apply to any account that is overdue, nor will we accept and process a new case until the account balance is paid in full and current.

*The parties to this Agreement mutually agree to waive any and all class actions in favor of mandatory individual arbitration of all claims arising out of, or related to, this Agreement.

*Atelie Digital Dental Solutions can amend the warranty, remake policy, and payment terms at its sole discretion and without prior notice.

  • Specialized in three types of full arch zirconia prosthetics: “Full Arch Zirconia,” “Full Arch Zirconia with Ti-base,” and “Full Arch Zirconia with Titanium Bar.”

  • Utilized CAD/CAM technologies to design and fabricate digital full arch zirconia restorations.
